Polymyalgia rheumatic (PMR) is an inflammatory disorder that causes muscle pain and stiffness especially in the shoulders. The symptoms are worst in the morning and are most commonly seen by women >50 years. Dr Yogesh and Dr Vaibhavi from Benglauru, Karnataka presented a report of a patient in the Indian rheumatology conference in Guwahati. A 58-year-old lady had muscles pains and stiffness in the shoulder. When she was investigated further, it was revealed that breast cancer was causing the symptoms. The warning symptoms were absence of early morning stiffness, poor response to treatment and progressive worsening of symptoms. The doctors concluded that high vigilance must be maintained for timely diagnosis of underlying cancer in patients who present with symptoms of PMR (muscle soreness and stiffness in the shoulders) .
